9/26/25 AJinerson - Called Trish to reschedule tech visit. Confirmed for Monday, 9/29 with 7AM arrival.


POC on site will be: 

7AM - Dr. B (Ken is aware of this) 

7:30AM - Trish will be on site too

Dr. B has key to let anyone in.

No other arrival instructions/requirements needed for this visit. 


Tech was on site today, 9/26 as scheduled, but was then informed by Olympic Vet, that this requires 2 techs on site. We will need to reschedule tech visit for 2 techs. 


9/12/25 AJinerson - Tried calling back and choosing different prompt.

Spoke with Trish and scheduled for Friday, 9/26 with eta between 9am-11am. Site can ONLY schedule on Fridays and tech is on PTO 9/19.

9/12/25 AJinerson - Received VM from Trish asking to call back at 205-678-8253. Called site to follow up and reschedule tech visit, received message stating "your call cannot be transferred"


9/11/25 AJinerson - Sent email to POC offering to reschedule to 9/15 with eta between 9am-11am.


9/11/25 AJinerson - Called site needing to reschedule tech visit. Was told my call cannot be transferred and to try again later. 


9/11/25 AJinerson - Received notice from FSE that this will need to be rescheduled due to personal matter, he already discussed w/ tech SUP.


8/29/25 RCook- Spoke with Daphne states they are needing to schedule for 9/12/25 as this is the best date for their facility. Arrival time between 9-11AM.


8/29/25 RCook- Spoke with Daphne, states that Jay is OOO today and all next week. Asked if there is anyone else available to assist with scheduling this repair. States that Trish could assist with scheduling but she is not available at the moment. States a Thursday or Friday would be best. Advised we have Monday 9/8/25 available or 9/11 and 9/12. States she will take my information and call me back.

8/29/25 RCook- Sent teams message to MFranks stating: WO-01300081- Olympic Vet repair in Westover, AL due 9/3/25- Kenneth Lancara is the closest technician to this site. Can this be assigned and scheduled for him? The next closest tech is Dylan Ingraham but his next available day is not until 9/8



8/29/25 RCook- Called Travis Lee/Olympic Vet, to determine if parts are being sent of if technician needs to evaluate first. States technician will need to go out and evaluate unit and call him and they will determine what parts need to be send if any. 

🔧 Work Performed
9/26/25 assessed unit and found spliced cables and unit was confirmed non functional. spoke with Tech support and they advised to replace the parts that were shipped to the site but it was a 2 person job due to having to disassemble the whole unit and it requires two people to hold top section . will return on monday to repair unit 9/29/25 replaced all parts shipped to the site (actuator, control module, and pedal) tested functions and returned bed to service

Hi-Lo Wet Table SN TBD Customer has lift table that is no longer working. Need to check connections and potentially replace controller, footswitch, or actuator. Contact Travis Lee for any questions - 541-501-3689 Before leaving the site, the FSE must check in with Olympic tech-support by calling 800-789-6867 and speaking with Andy Lake. If Andy is unavailable, contact Travis Lee at 541-501-3689 to report the service/repair status. For any issues, call tech-support. Parts ordering for the WO goes through Olympic. Out-of-warranty repairs will be billed to the end user. In-warranty repairs will be billed to Olympic Veterinary.”
