The parts are shipping Fedex Overnight and the tracking number for these parts will be supplied to Emsar when it becomes available. This is a RUSH Service call so please let us now the earliest you can get to the Redmond facility so we can inform our customer. Model Number: ESC2EYEST-X Surgi Eye Chair (Operating Manual attached) Scope of Repair/Service: Unit needs to be evaluated and if needed, diagnosis for repair. Unit giving faulty codes and then will stop working. When reset it will work again but the leg portion continues to not want to respond. “Through one connect it has been determined that either the cable to the leg, or the leg actuator itself is shorting out. We will have the service technician test the new actuator first, then if necessary, swap out the cable and/or the actuator”. The Emsar quote is under “Orion Eye Center”. This has been changed to “Deschutes Eye Center”. It’s the same facility location, no change. We do relies that we are requesting additional labor for this service call. No need to requote, just move forward with providing Repair/Service work.

09-10 JMorris: Arrived onsite at 1045. Customer (Kara) stated that the chair will be in use all day and won't be available until 1530 PST. Kara states they were having issues getting the request changed to afternoon service. On 09-09-2025 Michael Burris relayed technician's (Jason Morris) phone number. No call was received for change of eta request.

9/8/25 SReich - Per the attached email: The parts tracking number for this Service/Repair case number 00523426 is Fedex Overnight Tracking Number 457187866787.

Replaced the actuator. Tested the foot actuator multiple times with multiple program buttons. Arrived onsite at 1045.
