Description of Equipment Issue(s) and Service(s) Requested:
Case 124523 – Caster needs replaced, it is not rotating or rolling correctly. Will be providing prints for the brakes, we would like the field tech to look over from step one thru the end of the prints to make sure everything is installed correctly. Please read the instructions of install over fully, the single pedal braking system will not work properly if not installed correctly. Please call me before going into facility to make sure you have all instructions.

The signature and approval of the on-site contact that is worked with for the facility needs to verify by signature that they agree that the work to the product is complete and that the product is working per design. Please also print their first and last name, contact number and email address for MPI follow up confirmation as well the sign off. Do not leave the facility without this completed.

🔧Work Performed
Replaced the caster at foot end patient left. Went to hardware store to obtain new stainless screw after talking with MPI support. All casters now operating smoothly and all functions of chair function normally.
🔍Technician Findings
Caster at foot end left will not caster. One of the screws holding the caster in is bent.
