Fill out the Proof of Delivery (POD), get the driver and customer signatures
Fill out the Medical Delivery Observation Form

10/24/25-MBRAC-installed 3-225-002 chairs to site. DID THOROUGHLY CLEAN AND FUNCTIONAL TEST ON ALL THREE CHAIRS PER MIDMARK SPECIFICATIONS. ONE EXAM CHAIR SN# V2867133 HAS A SLIGHT TEAR ON UPPER LEFT SIDE OF UPHOLSTERY THAT MIDMARK NEEDS TO SEND SITE ANOTHER PIECE OF UPHOLSTERY.
