spoke with Bernie, and I gave him the WO# and told him a coordinator will call them to schedule the visit


11/04/2025 The address the chair is on is at 651 Dunlop Dr (Dr's Building next to hospital which is connected on first floor. Contact Gerald Green when enroute. Additional part being sent by Midmark and this chair is covered under warranty

🔧 Work Performed
10-21-25- I found multiple error codes to tilt and also found the over travel light was blinking on PC board. I tried to reset after but it would not reset. I contacted MidMark tech support and they had me check all limit switches and the switch the two over travel switches to see if it was the switch or the harness that was bad. A new harness is being sent out for replacement. 11/04/2025 - JWD Received parts from POC and replaced the electrical harness on the hubble board under the seat. Afterwards tested the tilt function which was still not working. Called Brian at Midmark and then removed base on back of the chair and the board was showing an error 0f 0403. Next took the multimeter and tested the switches on the board and the tilt fuse was blown which is a 5 Amp 250. Brian is going to order 2 fuses and number is 015-0346-22. 11/12/2025 - JWD Replaced fuse and plugged in chair and tested tilt position and it still wouldn't work. Contacted Midmark and spoke with Deann and she said to check the fuse with multimeter. After checking the fuse it had been blown. Next she had me to unseed the wiring from the hub board that connects to the actuater. Next I put another fuse in and plugged in the chair and tested it again and it still doesn't work. Deann again had me to test the fuse which had blown again. Then it was determined that it was a bad hub board. Deann will send out a hub board and fuses. Hub board part is 002-1749-00 11/24/2025 - Replaced the Hub Board and the Tilt Acuator. After replacing both parts put the chair in calibration mode. The chair went all the way up during the calibration but wouldn't come down and was doing a constant beeping. The chair had an error of OA which is an issue with the foot. Called Midmark and spoke to James and we went through the steps of calibration and also adjusted the silver round pieces that come in contact with the foot limit switches. After unplugging and plugging back. The rest was pressed while in calibration mode and the chair successfully calibrated with no beeping sounds. Notified POC of the completion of the repair

Hub board part# 002-1749-00 and Tilt Accuator part# 002-1731-01
