10/30/25 AJinerson - Received email back from POC Ivy:

POC on site: Ivy Johnson .  #386-317-3960

Arrival instructions: The address will bring him to the Lohman Building on the Halifax Health campus. Our office is located on the 4th floor, directly across from the elevator. 


Should he have any problems, he can call the office at the number.


10/30/25 AJinerson - Received email back from POC Ivy confirming tech visit for Tuesday, 11/4 with eta between 9AM-11AM.

Email back to Ivy confirming and needing POC on site and any arrival instructions for visit.


10/30/25 AJinerson - Sent email to POC Ivy offering tech visit on either Tues. 11/4 or Weds. 11/5 between 9am-11am arrival.

Local tech is on PTO 10/31-11/3 and this is the soonest we have availability.


10/30/25 AJinerson - Called Ivy and left 2nd VM regarding scheduling.

spoke with Ivy, and I gave her the WO# and told her a coordinator will call her to schedule the visit

🔧 Work Performed
11/4/25 AMW - Located exam table and confirmed issue. Foot section was the only function that did not work. Made sure the tray was fully pushed in, and foot section still had same issue. I removed the seat and the seat mounting cover to access tray limit switch. Found that patient right track for the tray had the bumper from the end of track in the wrong position keeping the tray from fully going in and engaging the limit switch. Re-installed track bumper and performed function test. Equipment passed all tests. foot section had full function. Notified POC of equipment status.
