11/14/2025 JRolston - Per email from Midmark, part shipped via Dayton Freight on tr# 32676416. ETA is Wednesday, 11/19. Updated work order accordingly.

11/13/25 SReich - Per the attached email: The parts for this SR are scheduled to ship out tomorrow via Truck. You will receive the tracking information once the part ships. Please reach out to the customer to schedule once the parts arrive. Have a great day!

11/12/25 CDempsey- this wo should have been put into Awaiting Parts Customer once it was learned that we were waiting on parts tracking. This would have avoided lots of issues and unneeded expenses.

11/5/25 SReich - Per the attached email from Midmark: Jack and Donnie have approved the ordering of the parts. The parts are scheduled to ship via truck. Tracking will be sent once it becomes available.

10/31/25 SReich - Spoke to Jack, gave him the WO # and told him someone would be reaching out to schedule.  

🔧 Work Performed
11/12/25 I Darrell called customer at 8;15 and was not able to get a message to the POC Jack Than. I hten called Kaitlyn to see if she ca verify the work order address of the chair. Katlyn advise to go to fort Sill and meet with Jan. I then went to building that GPS took me too and after 45 minutes in wrong building I was advised where to go. After I got in the building went to information desk and was able to get POC Jan Jenkins she then took me to the area where the chair was. I hten ask Jan if she had the parts. She state3d she had no parts and dont know where parts are. She stated that Bio med must took with them. upon getting in the area found that the room was occupied and had to wait on room to come available. I then called Jack and asked if he knew where the parts are he advised that he thought we had parts on the truck. I advised that Midmark sends part direct to the customer and that we dont carry parts on truck. He then said he had sent a email on the 5th for tracking g but there was no response from Midmark. He then asked me to look at the table of the rollers. so I went into the room and engaged the rollers to move unit and would not move. Still on the phone with Jack I advised let me call midmark and find out if I can get some tracking of the parts. I hten called Midmark and spoke to Brian he advised that the Quote was sign and and parts are in process of being gathered. And to be sent around the 14th to the customer and no tracking will be available unit UPS picks up the parts for delivery. I then called Jack back and advised him of the parts being sent and no tracking available. He then asked if they where being charged for the travel and labor I advised that will have to be taken up with Midmark. I then left facility and loaded up tool and was outside the gates when I got another call from Jack asking if I could go and look at the unit to see if anything could be wrong. I advised him that I had already left the base. He said since I going to be charged for the travel and labor you could at least look at the unit to see if anything else is wrong I advised that it would not be cost effective to take all the shrouds off and see if anything was wrong then have to put it all back together. That we should wait until the parts show up. Then Jack asked who was my supervisor. I advised to him that Craig Dempsey was my supervisor. Jack stated he was calling Midmark about not getting charged for the travel and labor. After the conversation I then called Craig Dempsey and advised That it wasn’t that I didnt want to look at the chair. No further action was taken and placing the work order in awaiting parts 11/26/25 I arrived on location of the Army Base it took me about an hour to get pass to get on the base. I then went to the facility for the repair. I then went to main desk to ask for Jenn Jenkins found out that she was not working. I then meet with April she was able to get me the parts for the table. When we went to the room the room was being occupied and I had to wait until room came available. I then was able to get in the room. I then removed the shrouds from the unit. I hten was able to look at why the table would not move when it was in the transport mode found that the caster wheels were crushed.. after review of the parts I was only sent the LH side move system swivel arm and not the right side. I then removed the upper shrouds that were bent and the n removed the LH and the RH outer shrouds and placed the bent ones in the box. I then started installing the shrouds in place. After installing could not figure out why the lower shrouds were not getting picked up by the tabs of the shrouds. I then figured out that a set of shrouds were missing. I then called MIdmark and spoke to Jason and advised of the missing parts needed to finish the repair. I was then sent over to dispatch to type up a quote for hte pair of shrouds and RH move system kit. I then called several time to Jack Than and advised him, of the parts that were missing and that he will need to sign a 2nd quote and we can come back out to finish the repair. I hten put hte table back together so that staff still can use the table. 12/22/2025 I arrived on location checked in at visitor station to get base pass. Went through the base truck was searched prior to enter y on base. Arrived on location meet with Jenn Jenkins went to dock got parts i the removed the shrouds on the unit after removing shrouds i then removed the move system swivel kit and installed new part # 002-100060-00 X1 i the installed all the shrouds and adding the #4 shrouds that was missing on prior visit. After installation i then function tested the unit and unit passed all test. I the took pictures of the unit showings the condition of the unit. I nthe go POC Jenn Jenkins and she came and function tested the unit and inspected the unit as well and all test passed by her. Unit was placed back in service
