🔧 Work Performed
Base was not responding to up or down commands. I nspected the limit switch, base actuator, and main PCB; all components tested within normal operating parameters. Fault was isolated to a defective hand controller, and a replacement hand controller has been ordered. 12/9 found chair to have shrouds completely removed. New controller solved the function issue but issue of shrouds falling still visible. Unit needed clips for shrouds but i had some in my person allowing me to add them to the shrouds. Began testing chair and found seat upholstery coming loose as all velcro strips ripped off of wood seat. called midmark and placed order for wodden seat with velcro strips. 1/21/26 Arrived on site and checked in with front desk and infromed them of repair. The got lead nurse and she informed parts not with them but after them looking they located parts. Once parts provided i noticed seat came with no velcro. Called midmark and after some investigating they determined they would send new parts and make note to include velcro 2/4/2026 Arrived on site and again checked in with front desk and again took some time to locate parts. Once parts taken back to room found that again velcro not attached to wood seat base. Called midmark to get size of staples. Spoke with supervisor and tried to locate staple gun at hardware store but after searching found none. Returned to site and removed staples from old base and manually hammered staples onto velcro to help customer use chair while we replace velcro. 2/10 Correct staple gun acquired so installed staples at home as compressor needed to install velcro on new wood seat base. took to sight and installed seat onto chair and returned to service
