ADDITIONAL NOTES
This dispatch needs to be completed on the same day as SR 1502692. This will be Zone 4 Dispatch to the EMSAR Inc,
YUKON, OK location. It is approx. 7 hours 30 minutes round trip. If a different service location is used, Midmark approval is
required before setting up service. If the total dispatch time, including travel, exceeds 8 hours, overnight accommodations may
be necessary. Midmark pre-approves these accommodations, and receipts must be provided when submitting the completed
SRA and invoice for reimbursement. If part(s) are identified after speaking with Midmark Tech Service, the Service Company
Tech must contact the POC for authorization before ordering. Once approved, notify Midmark immediately. If more time is
needed for the customer’s decision to proceed, inform Midmark Dispatch at 937.688.9175 or dispatch@midmark.com. For

12/4/25 SReich - Left a vm, gave her the WO # and told her someone would be reaching out to schedule.
🔧Work Performed
I arrived on location and found said unit. I then removed the four screws to the front display to get acess to the back where water hoses are. I hten function tested the hand pieces and as I got to the drill water was leaking from the back. I found that on of the caps on the manifold that screws in was loose and that where water was comming from. I was able to tight the cap tight enough to stop the water from leaking from it. Customer was present when i function tested the unit and no more water was leaking from that area. Fron panel was placed back on and unit was placed back in service. NOTE no travel was on site for another unit repair
