ADDITIONAL NOTES
Imaging/X-Ray Dispatch. This dispatch needs to be completed on the same day as SR 1502288. This will be Zone 4 Dispatch
to the EMSAR Inc, YUKON, OK location. It is approx. 7 hours 30 minutes round trip. If a different service location is used,
Midmark approval is required before setting up service. If the total dispatch time, including travel, exceeds 8 hours, overnight
accommodations may be necessary. Midmark pre-approves these accommodations, and receipts must be provided when
submitting the completed SRA and invoice for reimbursement. If part(s) are identified after speaking with Midmark Tech
Service, the Service Company Tech must contact the POC for authorization before ordering. Once approved, notify Midmark
immediately. If more time is needed for the customer’s decision to proceed, inform Midmark Dispatch at 937.688.9175 or
dispatch@midmark.com. For assistance, troubleshooting, or parts identification, contact Veterinary Tech Service at
844-856-1232, option 3. Thank you!

🔧Work Performed
I arrived on location and found said unit. Customer stated that hte unit was shorted out. I then plugged the unit in and powered unit on. I hten placed the sensor in the arm of the unit. I found that the sensor was not connecting to the software senor was not turning the icon from red to green. I hten checked the setting for the sensor and that it was not on the right setting I then put it on the DR sensor setting and then the sensor started calibrating automatically. I then created a test patient to do test on the unit. I had the tube head set to image the sensor on the unit and when I used the trigger to take image the unit wil;l not beep when trigger was pushed. I then pulled the fron display off to see if the trigger was plugged in and found it was to plugged into the display. After it was plugged in I then proceed to take image and unit was now working I was able to take 6 back to back images. Also customer was present when images were taken. Unit passed all test and was placed back in service.
