1/22/26 NSuarez for SRA date of service 12/18/2026 PG3 Serial # V1661442 There's should not be any notes that some type of work was done when there was no work performed, service done on that day only show date a tech went out was on 11/26 and that would have been Ryan Byer.

Had I known earlier that this would be a no go at all we maybe would have had a chance to maybe have Alan cover it today. Alan and Ryan are booked Thursday and Friday as well as Mike. 11/13/25 SReich - Left a vm, gave her the WO # and told her someone would be reaching out to schedule around the arrival of parts.  

🔧 Work Performed
11/19/2025 Mike Huffman. At 11:45 I called customer Ruko Bello at 773-554-9001. Advised I am on site at another location and will not be at his site today at 11:00am. 11/26: Shrouds are damaged. Documented the shrouds that were damaged and passed information along to Midmark to quote the customer for replacement shrouds of the ones which are damaged. Took the foot section apart. Attempted calibration of the table. The table raised to the fully up position and abruptly stopped calibrating. Power cycled, all functions present. But, table beeps while operating it due to it failing calibration. Customer had attempted to replace the i/o and power cables from the motor control hub pcb to the main board, but left them "loose" and not tied to the ribbon on the scissors. Removed and rerouted 3 cables properly to ensure no cables would be pinched during operation. Attempted to calibrate the table again. This time, it failed in the fully lowered position. Investigated limit switches on foot section per error codes. The limit sw. for the sliding portion of the foot section was loose in its aluminum bracket. Called Midmark, spoke to Deanne, who advised this bracket would need replaced, since the foot section was not making continuous contact during calibration. Customer came to check on my progress and advised they had ordered a new aluminum bracket. I replaced the bracket, holding it in place with one screw and one nut since I did not have any additional. Attempted calibration again. Failed again. Table always fails in the fully lowered position now. I then noticed that the limit sw. for the sliding foot section was a N.C. sw. and it should be a N.O. sw. Midmark to quote the customer new switch and shrouds. Hopefully the next visit, we will be able to replace this switch and get a successful calibration. 12/18/2025 Mike Huffman. This exam chair is in the hallway of Dcam3, OPGYN third floor. Biomed / Fred Douglas advised this is a fire hazard working on equipment in the hallway. Unable to determine what switch needs to be replaced. I called Midmark but they have closed for the day. 12/19/2025 Mike Huffman. Installed safety switch on foot assembly. All functions work and no error code on main control board. Attempted calibration which failed with error code 010F. Called Midmark provided SR 1493584. Completed trouble shooting. Midmark will send cable to foot switch. Part number 015-3284-00 and cost $116.00. 1/7/26 RJB -- This one is really MIDMARK'S FAULT. As crazy as that sounds, they sent the wrong limit switch. The foot extension limit switch was supposed to be white, not black. More importantly than the color, it was supposed to be NORMALLY OPEN, when the switch Mike had replaced (and was told was correct by tech support) was NORMALLY CLOSED. In both my conversation with the POC and Mr. Huffman, Midmark advised this was the correct limit switch for the foot extension. Anyway, Ruko had placed another order for the white limit switch and had gotten one in. I replaced this switch and attempted calibration, only to fail with another error code. Code indicated the foot extension overtravel was tripped. I talked to James at Midmark, who was very helpful in sending parts, and was sending a "foot actuator limit switch wiring harness". I was able to definitively prove this was the cause of the error. I was able to successfully calibrate the chair. Had MIDMARK SUPPLIED THE CORRECT NORMALLY OPEN LIMIT SWITCH, THIS WOULD HAVE NOT TURNED INTO THE MESS THAT IT HAS. I want to hold Midmark accountable for this if it's the last thing I do! 1/13: Chair has been function tested and was successfully repaired.
