11/30/25 RJB -- Did not have time to access this bed during service date of 11/26/25, the other serial numbers on this request required too much time. One of those serial numbers required parts, and a return trip will be necessary, regardless. Customer is aware. Midmark is not.

Please reschedule WO-01315228, WO-01315227, WO-01315224. All three are at the same location and scheduled for 11/19/2025. I worked all day and did not make it to this site. I have updated each work order with the following. Emailed DCarvalho, LYoder,CDempsey:I have no resources to cover this. Had I known earlier that this would be a no go at all we maybe would have had a chance to maybe have Alan cover it today. Alan and Ryan are booked Thursday and Friday as well as Mike. Alan has Inventory NW, Mike on PTO NW and Ryans next available is Tuesday. Please advise.

🔧 Work Performed
1/13/26 RJB -- Confirmed chair is working correctly and placed the covers on the chair. Returned to service. 1/7/26 RJB -- This chair is located in the basement. Chair was already disassembled by the POC. The issue was that the Overtravel on the backrest was tripped. I adjusted the paddle that trips this limit switch and was able to resume function. Verified all functions. However, there is a bent thumb screw to hold in the hand control holder. Requested a new part from Jason at Midmark. New part being sent. 12/18/2025 Mike Huffman. This exam chair is in the hallway of Dcam3, OPGYN third floor. Biomed / Fred Douglas advised this is a fire hazard working on equipment in the hallway. Unable to determine what switch needs to be replaced. I called Midmark but they have closed for the day. 11/19/2025 Mike Huffman. At 11:45 I called customer Ruko Bello at 773-554-9001. Advised I am on site at another location and will not be at his site today at 11:00am.
