8/4/25 RCook- Spoke with Mayra, rescheduled for 8/28/25 with arrival time between 12-2PM. Also scheduled for themorning of 8/29/25 with an arrival time between 8-10AM. See logged call on WO-01255871

6/20/25 RCook- Sent teams message to CDempsey and DCarvalho stating: For the Vitas Healthcare-Villa Rosa Invacare PMs that are left for Zachary I know that you said Zachary will need an overnight for this return trip. My question is do you want me to move forward with getting approval for this return trip from JJ or do you prefer we eat the cost on the return trip? I imagine that the first trip will not be as we did not request approval beforehand.
Waiting for response before moving forward.

6/19/25 RCook- Sent teams message to DCarvalho and CDempsey stating: Craig Dempsey David Carvalho I am working on scheduling Vitas Healthcare-Villa Rosa Invacare PMs for Zachary Stieben. There are 5 remaining WOs. This is 5 hours 1 way. Do we want to keep him overnight again? My concern is I don't see that we requested approval for the travel the first time and I don't think they will cover the travel without approval.

3/28/25 CDempsey- This is a hospice facility and bed is in use. Facility manager to contact coordinator when available.
